Uniform Urban Developments COVID-19 After Sale Service Authorization Process

Questionnaire to be completed by all home owners prior to booking an in home service appointment.

This webform is mandatory for all home owners who live in a Uniform home/community and wish to commence service work in their homes. This enhanced screening will assist in creating a healthy and safe environment for all homeowners, our staff and our trade partners. Please note our technicians and trades have been asked to review the same questions in screening their staff in preparation for work – to reciprocate the effort.

Have you travelled outside Canada in the past 14 days? *

Do you currently have any of the following symptoms: fever (over 38° C), coughing and/or difficulty breathing? *

Have you been exposed to a person who has a confirmed or probable case of COVID-19 infection? *

Are you or is anyone in your household currently self-isolating? *

Do you agree to respect the Social Distancing requirements of 2m/6ft allowing the trades person/s to complete their task?

Any trade interference or harassment, or violation of the Provincially mandated social distancing protocols, will be taken seriously and will result in the halt of work in your home or on your property.
